  2. Select your state and enter your first and last name (required).
  3. Enter your city and ZIP code (required).
  4. Enter your email and complete the email-validator step — MyLife sends a one-time code, valid 24 hours.
  5. Enter your birth year (required — it helps them locate your correct profile).
  6. Optionally add a middle initial, maiden name, or your profile URL.
  7. Complete the reCAPTCHA and submit.

Gotchas

  • The buried form. MyLife's homepage footer has no direct opt-out link, and its privacy-policy link is broken — /privacyrequest is the working path.
  • Account closure side effect. Submitting a removal also closes any registered MyLife account tied to your info and unsubscribes your email.
  • Alternative channels. You can also email privacy@MyLife.com or mail a written request (see MyLife's form for the current address).
  • It can reappear — re-check periodically.
