- Select your state and enter your first and last name (required).
- Enter your city and ZIP code (required).
- Enter your email and complete the email-validator step — MyLife sends a one-time code, valid 24 hours.
- Enter your birth year (required — it helps them locate your correct profile).
- Optionally add a middle initial, maiden name, or your profile URL.
- Complete the reCAPTCHA and submit.
